Scheduling Procedures & Insurance
Positive Nutrition of Omaha works with insurance and is currently in network with Blue Cross/Blue Shield, United Health Care, UMR, TriWest, and Aetna. We recommend using your insurance. To do so follow these steps:
1. Complete Referral Form
To schedule an appointment you must first download and have your physician complete the referral form and fax to 402-243-6606.
2. Schedule a Session
Once your physician has faxed the referral, schedule using the button below:
